I AM READY!

LASER has a program to assist in emergency readiness.

  • You can download the Fillable Intake Form above, take your own photos and embed them in the form.  Instructions are provided.

  • LASER can do this for you if you need help.  We will prepare your Intake Forms before disaster strikes, with photos of your equines, and provide copies of the shelter rules and waiver.  When you must evacuate your home, you can be ready to present these documents at the shelter. This will expedite the intake process, meaning less time waiting in line, and ensure accuracy of animal and owner identification.  Contact us through our Chat button on the bottom right of this screen and we will arrange a time and place to do this for you!

These darling crochet horses are for sale at our community event booths.

OR YOU CAN PRINT YOUR INTAKE FORMS & COMPLETE BY HAND

Complete Section I of the Intake Form and LASER volunteers will do the rest at the shelter site.

To embed your own photos, see I AM READY! above.
